"Settlement", not individual transactions. Each day merchant banks have to settle with VISA for the aggregate amount. So we are talking about tens of millions of dollars worth of transactions for even the smallest merchant bank and billions for the largest. Paying $10 in ETH gas to settle millions of dollars is nothing.
